I have never seen a piercing wound so awful before. Its not normal that's for sure but we cant tell Chris shit were just damn dirty trolls to him! I am sure this has been asked in the 100 pages but I havent seen it asked yet. Anyone with medical background or with piercings, know what could happen if he just leaves it? There's no way he can keep it clean enough for it to heal on his own.
Pages 75-78 have some good medical opinions on this matter.
 
No, I've just done a lot of caring for people with chronic health problems and some nasty wounds have been among them. Advances in wound care technology never cease to amaze me. There's a "smart" dressing under development at the moment which will even read wound temperature (!).

How does the whole referral to plastics thing work in the US for someone in Chris's situation? Here, the government will pay for it if plastic surgery is needed to restore function or repair "disfigurement" but not if it's for purely "vanity" reasons.

And yeah, outpatient wound care clinics and community nurses doing wound care in home is an option here but I'm not sure what is and isn't free in the US.

Here's what you need to know about healthcare in the US with regards to your statement.

1. For all minor things that can't be treated at home, you go to a GP or family doc. They give you instructions, like how to pack a wound, and send you on your way.

2. In a community as small as Ruckersville there may be a community nurse, but that's doubtful. Nurses aren't usually allowed to practice on their own here, the LNP being an exception. We don't have enough nurses as is. And they tend to prefer working for a hospital or under a physician.

3. Outpatient wound care may exist here. Definitely not in home. Doctors and nurses outside of very small communities rarely or never make house calls. Or they make unofficial house calls, like you're a friend of mine who needs sutures, I have a suture kit, I come over, and save you time and money by sewing you up. It's an "I'm a close friend" kind of thing outside of extremely small communities.More likely, considering that this is America, the chandlers would have to make an appt at the physician's. If they see a family doc, they can forget trying to go see them with any regularity. That's impossible, considering the volume of work that GPs in America do. Anyway, the doc would tell them "Here's what you need, here's how you do X, Y and Z. Good luck. Call me if you have any problems." After that, you're pretty much on your own.

4. As for referrals, a GP goes "Welp, I can't treat this, or I ain't comfortable with touching that. I'm going to write you a referral to a plastic surgeon" Medicaid certainly would not cover plastic surgery for vanity reasons, like a taint hole. the surgeon would have to be on the list of government approved surgeons. Because medicine is not government funded/run. It's all privately owned. It would be less of a pain in the ass to let the taint heal, compared to the amount of work necessary to get the scarring on the taint fixed.

By the time Chris's taint turns black from gangrene, I doubt he would ignore it and let him self die.

Basically this, if you don't have socialised medicine from the military (still super rare, I only know one person who has gotten it.) and you aren't dying and on hospice you aren't getting a house call. If he had a tard handler they might help him care for it (like Amanda Baggs, fake autistic extraordinaire) but otherwise he's on his own until he gets so bad he has to go inpatient. I'm banking on him getting so bad he has to go the hospital and maybe get sent to a rehab facility to care for it until it's healed. (Usually in cases where someone is in a situation where they can't take care of a life threatening injury or wound, medicaid covers this.)

Generally in the US, Plastics (but sometimes Gen Surg or even Ortho) handles these large wounds. They often take quite some time to heal, and can possibly require scar revisions down the road.

I'm not sure of Chris' insurance status (Medicaid or something else?), but I can't imagine he would have any problems from his insurer. This is the most appropriate referral for this wound, so it's medically justified. This is going to take some time to heal, so he will have a number of appointments to check the wound and see how its progressing. There is definitely a risk of this thing getting infected so close follow-up is key.

Since there's these risks that could potentially require surgical intervention, it's usually best to just have Plastics handle it from the start. This is usually way beyond most GPs comfort level.

That's what I figured, although I assume if he actually needed hospitalisation due to infection plastics might get called in.

I never really thought about the lack of community care options in the US. Most discussion of health care centres around hospital costs.

Here you can have a GP come to your home after hours at no cost and people still go to the ER because they don't want to wait an hour or two for the doctor to come - then they walk out of the ER because they haven't been seen within 30 minutes.

Plastics might get called in. More likely General surgery. Plastics is reserved for those who can pay a plastic surgeon to see them up and make them look pretty, or when something involves working around nerves.

Off topic, but yeah, we tend to have poor community healthcare options. What ones we do have are demonized by 50% of the population as some sort of horrible socialist thing that does something or other to ruin the economy and encourage freeloading.

You got it right. Unless Chris is not getting better, or getting worse, and they call the clinic's advice nurse, they're being left to their own devices. Once you're out of the clinic, you're on your own more or less.

I'm sure his regiment of smearing expired Oestrogel and hand sanitizer on it will keep the nasty bugs at bay for decades to come.

I knew a guy who has chronic recurring taint abscesses. When he gets them he has to go in on a near-daily basis to have the bandage pulled out of the wound, cleaned and repacked with bandages so it can heal from the inside out. It's a long, painful process until the healing is done. I think Chris would be bitching about it if he were undergoing something like that.

Chris would dismiss the pain as natural. He's growing a vag, of course there's going to be a little pain !
