Except that individuals with CAIS still aren't normal women with Y chromosome. They will look like one sure but internally they aren't and this includes their hormones production. They still have balls, internal ones but still balls, and those produce testosterone and other hormones like in males. Do their condition that testosterone eventually turns into into estrogen. This why they look women and get many female secondary sexual characteristics but because this still isn't actually female hormone production so don't get full female puberty. Naturally they are infertile but also lack adult body hair, have bridle bones and few other more subtle things like lacking acne and having larger teeth. Their gonads also have significant cancer risk and might need to be taken out resulting that they have go on HRT.

Don't get me wrong there is no reason to not to treat these people as women in all most all circumstances. They grew up as girls, they look like women and most seem to behave like normal women. Mainly just their medical needs are different do their condition of being males with a birth defect.

Yes, that's all true. The important part is that they are born with female genitals and a vagina. So they are born and live as women, albeit with a severe medical anomaly. But they (and other men & women with various intersex conditions) have ZERO to do with medically normal males with fetishes on parade.
