I was wondering this personally. He is at high risk for them given that he's very obese and largely sedentary. His skin also looks like his mom is shaving him given how irritated it is. Ingrown hairs are another risk factor. Nasty, painful cysts those are. They tend to smell terrible, and have a high reoccurrence chance even after removal. Just like Yaniv himself, really.

For starters, tampon strings are typically white, twisted cotton yarn - though I've seen blue and red ones before - and they do not taper off at the end. Some even have a knot / mini-tassle (supposed to help you grab them more easily during removal without your fingers slipping off the end).

He's just exceptionally oddly shaped.

Naw, a pilonidal sinus looks more like a small hole leading into a tunnel. They are unbeliveably gross when they do become infected or a pilonidal cyst forms instead, but that ain't what's going on here if this even is an ass, and I am having my doubts.
