Shady foreign surgeons don't have long wait lists, they don't have requirements for counseling or informed consent or living in your "preferred gender" for any amount of time, etc. And a real, reputable surgeon will want you to show that this isn't a fetish-driven impulse decision, which most of these sickos can't do because... it is. And the reputable surgeons also have wait lists, cost more, and so on.

Basically troons are fetishists who are desperate to live their fetish out, so they don't want to wait and save money, or wait until a good surgeon has an opening for them, or risk having to go through counseling or make any actual effort before getting their dick ripped open and turned inside out, so enter the opportunistic foreign surgeon who doesn't want them to do anything at all except fork over some money!

This. The NHS have such a strict requirement before they do any kind of evasive surgery like SRS. you have to have lived socially in your gender for more than two years & have a BMI (body mass index) of 25 or less and tons of other requirements, psychiatric evaluations... all that stuff.

Maybe surgeons in Thailand or Turkey or wherever are held to less stringent ethical standards than in the West. Maybe I just don't understand medicine, but I don't see how any doctor could perform sex change operations, as there's nothing wrong with a human's body that demands the amputation of testicles and penises in order to use the flesh to fashion a franken-vagina where those appendages used to be. Or the inverse for cutting out of a vagina to affix a franken-penis. Just on the basis of ethics, it should be unethical for any doctor to perform medically unnecessary surgery. I can see how the most ideologically committed doctors and surgeons might be willing and able to manipulate the system to perform these procedures in advanced countries, but even with these doctors and surgeons there will be at least some cases where they are unable (but not unwilling, probably) to do so. Hence such patients having to go to Thailand or Turkey or wherever.

in turkey, for example, you have istanbul. huge city, millions of inhabitants, lots of foreigners, large population (thousands, possibly tens of thousands) of english speaking cosmopolitans in business and academia. that's enough to serve as the kernel for it, from those people it can then spread to their family and friends. it won't be a large number, but it's enough to result in a handful of troons per year.

and there absolutely are troons in algeria or ghana. again, much less than in the west, but they definitely exist. even without cultural spillover, you would still have that one in a million deviant who went online, found tumblr on his own and fell down the rabbit hole from there.
