The first steps to improving yourself is getting a job so you can get work skills, your own income, and possibly meet people to talk to in real life. There's no reason to be afraid of a transphobic hiring committee since you live in the 16th most progressive city in the nation, and you can pursue a case if they do turn you down for being trans. But most importantly is that you need to see a doctor for your foot, there's a definite risk it may become something worse if you put it off. You'll be amazed how much your life will improve once you start looking after yourself.
By the way, I'm not saying I sympathize for you with this. Just that you really should consider why multiple different people across multiple unrelated communities keep saying the literal same exact thing to you.
