Dude neutered himself without any kind of painkiller. My diagnosis is he's as crazy as a shithouse rat so trying to logically discern what's going on up there might be a pretty pointless endeavor.
Also I'd bet real money on that he ripped his stitches himself on purpose to get back at the hospital in some way for how he felt mistreated/not showered with attention/asspats enough. Complications for them are actually likely related to that, not necessarily that they ripped their own stitches though that is totally possible. Depending on the scarring and how much he took off himself when he did it / what damage he caused to the area. The testicles supposedly form the 'labia' of the pseudovagina so it's not surprising to have complications when they were already working with less material. Same thing with Jazz Jennings and his micro/undeveloped penis.

I’ve heard of it as a measure of suicidality. The theory being that there are all sorts of moral and practical weights attached to admitting suicidal thoughts and plans. Whereas the question ‘if you could push a button and die instantly and painlessly, would you do it?’ removes those concerns and is therefore more able to focus the underlying mindset. In theory anyway.
