Ironically, it was this type of reaction that Dr. Dobsyn was trying to prevent. When he told Gwen that Claire would likely die before a year, it wasn't to be cruel, but it was because he knew that whether or not she died within the year or lived longer, her condition was almost certainly going to cause her death at a very young age. He wanted Gwen and Scott to be prepared for the inevitable, so that they wouldn't be so emotionally distraught when it finally happened.
Gwen tries to make it seem like Dr. Dobsyn considered Claire's life worthless and that he acted as though she might as well be left to die because he said he wouldn't judge her and Scott if they decided not to place a feeding tube. Yet, he still offered her medical intervention,(which Gwen refused)so it wasn't a matter of him acting like Claire wasn't worth saving. Whether he believed it or not, he still gave Gwen and Scott all the information he had so that they could make an educated decision on whether or not to continue life saving measures for Claire.
In 2013 Claire and Lola were diagnosed with Asparagine Synthetase Deficiency, a condition that gets progressively worse over time and is almost certain to be fatal, either directly or through secondary causes like viruses, aspiration of food. etc.
Yet somehow Gwen managed to live in such denial that when Claire finally died , she was shocked and devastated. She really seemed to think that her spuds would live for a normal lifespan. Now Lola is about to meet her maker; even if she somehow survives this hospitalization, it'll be the next infection or the one after that that kills her. It's inevitable. Yet, once again Gwen is acting as if she never knew her daughters had a fatal condition.
