The wide smiles also show that he's a money-fixes-problems person with no self-awareness. A normal way to behave would be underplaying them hard and if anything treating them as a new normal for yourself. This makes the change less jarring and shows that you're already accepting that there are new issues to work on and these are just one step of a path to self-improvement. To him they are a tool to draw attention to himself, show how great he is and how criticism of him is invalid, how he so doesn't care and haters make him famous, etc. The only reason he is smiling is to show his fake teeth which makes it additionally insincere. He thought teeth would be like a video game character upgrade, +20 to Dignity or something, but it's just another way to make him look like a middle-aged fool.

I love that he wants people to admire him for getting new teeth. All those things imply is that you spent decades being complacent about having horrible dental health/hygiene.

well he's not losing we know that, otherwise we'd be subjected to endless tweets about it. He's plateaued at 350lbs which means the benefits of the surgery have already been negated by his eating habits and his stomach is stretched enough to consume the 3K or so calories a day needed to maintain that.

it would be awesome if he did a body composition test. I'd hazard a guess he's still well above 40% body fat. He has the lean mass of a 8 year old Sudanese child.
