Photos are 8 months post OP.

Surgery was Penile-Inversion Vaginoplasty with Dr. William Boysen (Urologist) and Dr. Shailesh Agarwal (Plastic Surgeon) at Brigham and Women's in Boston.

Complications:
A few days after being discharged, the vaginal lining fully detached and herniated through dehiscence below the labia. In the ER, Dr. Agarwal removed the lining and ordered the discontinuation of dilation, causing the entirety of the canal to collapse closed and outward. The team assumes that this issue is due to a urethral tear which caused urine to drain into the wound site. The perineal wound dehisced significantly over the next few weeks and required corrective surgery to close after two months of self-administered wound-care (as directed).
Prior to the collapse, canal depth easily exceeded 5 dots on the purple dilator.

Results:
Visual:
- "Labia" exhibit testicular musculature activity & constrict/relax with temperature.
- Canal entrance entirely collapsed; bunched up tissue present bleeds intermittently
- Urethra ends inside a small pouch behind the perineum
- "Clitoris" retains distinctive penis head shape, is above labia, and is visible when labia are closed.
- Clitoral hood absent.
- Pubic hair growth extends to the clitoris.
Overall, the vulva has an open "V" shape, closed and hanging loose at the back but fully open and taut near the clitoris. The mons has fat deposits that abruptly end.
Sensation:
- Clitoris retains partial touch & sexual sensetivity
- Labia retain pain sensetivity only.
- no internal sensation (excluding "pressure" sensation due to musculature)
Functional:
- No penetration possible due to the removal of the vaginal lining & subsequent canal collapse.
- Urethra placement causes urine to exit to an enclosed pouch and then dribble out. Very messy; stream strength has to be carefully controlled and cleaning is intensive.
- Pubic hair being right next to clitoris causes sensory issues and makes shaving really scary.

i'm considering a PPT revision with the same urologist and a different plastic surgeon (dr. david o'brien-coon). most of the other doctors i've talked to either flat out don't do revisions on other people's work, or have a waitlist of many years.

Testicle skin exhibits a lot of independent mobility in response to temperature, because in order to remain viable sperm need to be kept at a slightly lower temperature than the rest of the body. I have always been curious if cutting it up & rearranging it would change that at all. I guess it does not! No wonder there are so many problems with this surgery, its like if you had to sew a garment using fabric that changes in size repeatedly. It is notoriously cold in hospital operating rooms so the ubiquitous wrinkly labias might be a result of surgeons sewing up constricted skin. This likely means that the better vaginoplasty results we have in this thread are dependent on whatever temperature the photos were taken at

I don't think this nigga is going to be able to do PPT. The "closed" canal is scar tissue. god only knows what shape and extent of scarring is in his pelvis now. If it is stuck to anything important it is so very over for him. He should get a revision to slice the piss-pocket down the middle so his pee can drain. The up-and-down movement of the scrotum and the wrinkling/unwrinkling that accompanies shrinkage (the things that thermoregulate the testes) are mostly due to structures called the cremaster muscle and the Dartos fascia, respectively.

I didn't think the cremaster was used in any way in a vaginoplasty and am now off to read some horrible papers by horrible surgeons to try to discern how on earth this could possibly be happening. Also, every day we stray further from God's light.
