As has been pointed out, it is his ego, but it is also that he imagines himself to be this powerhouse marathon runner.

I would pay for Fatrick to get an MRI and whatever other brain imaging needed and have a professional analyze his medial prefontal cortex, the part of the brain most associated with our sense of self, self awareness, and processing any thoughts related to our self image. If it's bloated, shrunk, or otherwise fucked up is a burning question at this point since he is absolutely unable to accept any losses or harm to his image such that he is able to fabricate alternative answers in the few times he's been confronted and bothered to give an answer and I am astounded at his delusions. At this point I don't want him to do drugs and suffer any sort of ego death because it's impossible. I think his hardware is fucked such that it is an actual limitation on his mind and its capacity for reflection.

Having carefully reviewed these photos, I have a couple of observations I haven’t seen mentioned yet.
1. In none of these, or any photos over the past few years, has Patrick had a friend in the picture as well. Hell, even Nikki makes very few appearances. I’ve thus come to the conclusion that he has no friends and that his frumpy wife does not like being around him.
