I'm starting to consider that Elliot's actions could be partially explained by side-effects from improper use of medication. He doesn't show any signs of schizophrenia or bipolar personality disorder. I don‘t believe for one second that “plural system” story. He shouldn't have taken such strong drugs. He‘s clearly autistic and probably narcissistic, but that's about it. I'm beginning to doubt even his supposed eye condition.
I think he invents those diseases to get from strangers the pity and care he believes he doesn't get from his family. Maybe he likes how those medicines feel and is using them as recreational drugs?
How likely is it that an autistic bipolar transexual narcissist who is into BDSM and has a plethora of other diseases even exists? Remember that some of these conditions are genetic, meaning that his parents or grandparents would‘ve needed to possess them in the first place in order to transmit them to Elliot, but if that were the case, they wouldn't have managed to have children or take proper care of them.
In the rare instance that such a human could even be born, he would be completely incapable of functioning. He'd never have gotten a job at Google, much less kept it for so long. He'd be dependent on his parents for the most basic of tasks.
The only explanation that would allow for Elliot's existence is if he's lying about all conditions he admits he has (correct me if I'm wrong but I don't remember him ever admitting the most obvious autism and narcissism diagnosis because it would really damage his reputation).

He's a gay slut that has predispositions towards sadism while also needing the approval for his chosen lifestyle. He didn't get that from his family, and as is typical for the 'alternative lifestyles' club, he created a proxy family of older men.
