The penis delusion is more likely caused by the real physical discomfort she's had in her genital region for many years than a thought disorder. She wants to believe she's some kind of mutant boy who was mistakenly born with female parts instead of accepting that she caught an STD from one of her loser FWBs back in the day and refused to properly treat it. (Or she developed a cyst or a polyp of some kind by no fault of her own but still refuses to get it checked out.) Her junk is constantly bothering her for real physical reasons like recurring infections as well and she's coping by being trans instead of a responsible, hygienic adult of either gender. The dying obsession is definitely all mental, though. She knows how hopeless she is and is probably constantly having panic and anxiety attacks, making her feel like dying. All the crazy shit she says in regards to her body all sound like health anxiety gone completely out of control filtered through a below average IQ, and a compulsion for attention seeking, rather than a form of schizophrenia. But that's just my opinion.
I don't think her mother could do that at Burger King or Whataburger.

Granted this was over a decade ago and in a large city, but when I applied you had to pass two separate training courses before you would be hired even got to the actual hard training. The first was listening to various calls and accurately transcribing the information fast and efficiently (nothing traumatizing but various voices at different speed and volumes and getting the correct address and info typed up). The second was dispatching the closest unit to a call based on the priority of the call and what the unit was currently on and keeping up with what the unit was telling you. You didn’t even get hired and to the months longs actual traumatizing shit until you passed that (I got a job I wanted before then so I fortunately never did).Bump & Grind!

Funny you should mention 911. Pixy actually applied for a 911 dispatcher job years ago and failed training hard. She couldn't emotionally deal with serious emergency calls. I'm not sure what crimes they use as examples during training. Rape? Terrorist attack? Murder of a child? They probably throw everything they can at you to see how you hold up. It's understandable if you can't handle it.
But that wasn't the whole issue. You see Pixy needed extra snack breaks. And she didn't understand why a 911 dispatcher couldn't have a break any time they wanted. She thought it was mean that she couldn't have breaks on demand. Because that person with a gun to their head can just wait. Pixy needs to scarf down a tray of mochi. She has low blood sugar.
Unless Whataburger is putting meds in her food I doubt it.
The idea of Debbie following Pixy around and trying to throw her meds into her food when she's not looking is kind of hilarious though.
