Nobody here is mocking the physical degradation that naturally comes withing aging. If Nick were getting some wrinkles, or some grey hair, or thinning hair, his appearance would barely warrant a mention.

What you are seeing in him is not the normal effects of aging. The man looks like he's dying of AIDS and alcohol poisoning. The difference in his physical appearance between now and even two years ago is genuinely unsettling. If you haven't noticed that, you're not paying attention.

As for Kayla, I think the big talking point with her is that Nick gave his rant about her body being destroyed, and then she started looking radically different. Rightly or wrongly, people are speculating that Nick might be encouraging or forcing her to change for his own gratification.

Even 2022 wasn't too bad, but you can definitely tell she's been very comfortable with others at that point (post-Hedo II). Also in my opinion, 2022 seems to be before all the alterations began, and it was all completely downhill from there.
