Jesters were supposed to be able to criticize the monarch or emperor but be taken in good humor.
They were an early form of satire in certain regards.
Fat is not a Jester. He is a loudmouth shouting treasonous things from the pillories.
He is largely ignored however as he is clearly a village idiot that probably got brought along by a village to the city when the village was destroyed by marauding enemy soldiers, or fleeing a plague.
The village folk might have abandoned him due to concern that association with him would get them all hung, but the city burghers took pity and let him stumble around getting drunk in the various taverns, providing entertainment for the rude mechanicals and off-duty guards who like to fling spoiled pepperonis at him.
