Considering his tactic of opening his mouth and going after people's girlfriends/wives, my guess would be after being kicked out of a bar or getting into an argument, he opened his mouth and crossed the girlfriend/wife line. It's Mediterranean Europe, family and loved ones are a top priority, moreso than many other cultures. Mouthing off how you are going to beat them down is another one. Ralph does both frequently.
 
I've always wanted to know how Null truly feels knowing that he made another human self destruct in such amazing fashion all because of one corn joke.

Imagine having that kind of real estate in another person's head; you crack a single joke and in response the target of your mild ribbing proceeds to self destruct for over a year without any signs of slowing down all while screaming curses to your name.

I think it was one person, and a person who Ethan is ashamed of getting so thoroughly beaten by, or he wouldn't be so insistent that there was more than one person while also not being able to keep his obviously false details consistent
