Trans suicide rates are high even in very liberal countries like Sweden. What WILL they blame here?

They blame it on "transphobia". Things like lesbians/straight men not fucking them, not being catered to by society, not getting free surgeries, etc.

It tries to close up because of the pressure of the pelvic floor muscles around it, which maintain a constant low-level contraction in a normal body. Also probably something to do with scars contracting, I’m not sure about the exact anatomy of it. But it doesn’t “heal” the way a wound does. Don’t quote me on this, but I think that when troons say they “lose depth” it means (sometimes at least) that the tissue has tightened to the point it can’t be penetrated with a dilator rather than that it has truly “closed” at the top/superior end. I am not an amhole expert though.

And you are right, the inverted penis is simply not a wound. It’s lined with skin. A comparison would be something like a deep belly button or an armpit (assuming you couldn’t lift your arm). That’s probably a large part of the hygiene/smell issues: you have a deep skin pocket that is not self-cleaning like a vagina is and which a lot of troons don’t even clean and maintain very well.

My one hangup with this line of thinking is: neovagina owners are told to dilate less frequently as time goes on. If they close because of the pressure of the pelvic floor muscles, wouldn't they have to keep dilating at the same frequency for the rest of their life?
I'm not a doctor, but I do think that it heals similarly to a piercing: the longer you've have it, the less often you have to put jewelry in the hole to prevent it from closing up.

Exulansic pointed out that these dilation shedules seem to be or are experimental and that dick butchers have different dilation shedules. They don't know if the reccommended shedule is the one their butchered costumers need. Also no matter how often one dilates, the Amhole loses depth over the years and some even shortly after surgery as we have seen in this thread...on the other hand we don't know if the troons dilated like they were told or if they did, perhaps their Amhole needs a special shedule and their butchers just don't know that.

Perhaps every Amhole needs a different shedule depending on how their body reacts like scar growing etc...but it's impossible to find that out.

Don't multilate your dick. That's all it takes to avoid this nightmare. These idiots think it's either Amhole or suicide when actually they have the choice between getting help for their mental illness or genital multilation & suicide. Not talking about children here of course.

Patients who want elective, untested surgeries are normally put through some paces with psych consults, medical compliance testing and histories. These patients are not. The surgeons involved do not care about the outcome, just that they booked another cash cow.

There are boards of ethics and regulatory groups that the patients can report to, but they won't. If they do, they may lose access to the only doctor that will try to fix the fuckup. This is why they don't like to do revisions on other surgeons' mistakes. It keeps the patient from reporting anything to any place that could stop it.

Educational video about creating the space where the neovagina goes. I have to link and not embed because it is age-restricted. It's pretty physically brutal although the tools used are interesting (one looks like a crowbar!).

This video is about lining the neovaginal canal. I think different surgeons use slightly different methods (this one uses a scrotal graft as well as penile skin for the inside, I don't know how common that is) but the short answer to your question is that the penile and/or scrotal skin seems to be stretched out when lining the inside of the neovagina so that it is much bigger than it looks when it's just sitting on a penis or a scrotum (see spoilered image below). I skipped around a bit in the video but it probably makes more sense if you watch it beginning to end.

(Edit: I know we are all grown-ups here and should not need warnings, but for any idle lurkers, please only watch the videos if you can handle that kind of thing. They are very graphic and I don't want to be responsible for anyone passing out when they see penises getting skinned)

Stretching penile skin seems to be a very bad idea. It's not that thick and there aren't any muscles or fat under it. No wonder that some troons end up tearing and shitting through it.
