IIRC it's just a relic from the late 2000's when some major studio released a surprisingly well-made Brady Bunch porno parody called "This is Not the Brady Bunch" with the first part of the title in very small font, thus making the DVD cover look like it just said "The Brady Bunch", and this video was super controversial and got a lot of attention. From there the trend continued and kinda snowballed, and the trend still exists.

Not familiar with any of these titles. But "Diagnosis Squirter" is a delightful throwback to a day when people pretended that porn had to have a concept/story and needed a bad pun for the title. I approve.
