Having studied many pics of the docs cycling apparel I am sure there is something being worn under the skinsuit. This is not normal practice and if i am right it is for two reasons. The doc uses a conventional saddle and maybe the SRS needs extra protection, even though time on the saddle is minimal, or it is some kind of underwear performing a tuck of cock and balls.
It's padded. All road and more performant shorts are padded or worn with padded undershorts. Even real women can look like they have a dick.

I am not reffering to the padded area. I am focusing my attention on what appears to be an elastic band of some sort at waist height which can be seen in quite a few pics. In another i am sure there was something under the shorts at the hip. These things are under the skinsuit and not normal as underwear is not normally worn.

I only really do distance cycling so maybe someone more experienced in track could chime in, in my experience double and triple layering is pretty common to soak up sweat and cushion your perineum, seat friction + sweat can leave you bleeding

Then again most of my rides are 40 miles or more split between roads and rough trails

Around about the 1980's the padding in cycling shorts switched from chamois to man made foam/gel padding with a non-lycra surface to sit on. The switch from chamois to other materials was due to chamois needing to be looked after very well, post wash, and the anti-bacterial qualities of the man made stuff. This padding comes in various thicknesses to suit different disciplines or rider requirements. Triathlon racers will have no padding or the very thinnest as they ride after swimming whilst a tourist/audax rider may use the thickest padding.
Saddles have changed alot in the last decade. Early cut-out saddles were a bit naff as where the cut-out stopped, towards the front of the saddle, the edge there would put pressure on the perineum. Nowadays cut-out saddles have the cut-out extending right to the nose. some saddles have no nose as such but two prongs for the front of the saddle for maximum relief. See the ISM Adamo range of saddles, for examples.

There is alot of confusion about when saddles are discussed regarding widths.and fits. Human sit bones are only sat on when you are on an upright city bicycle, or sitting on a chair. When riding drop bar bicycles you are sitting on the pubic rami area of the pelvis. The modern ergonomic saddles that are noseless or have extreme cut-outs put you firmly and squarely on this part of your body so no more perineum pressures.

The golden rule with clothes, for saddle comfort, is no seams and no materials bunching up or sliding against the skin. These along with cleanliness are the best way of staying clear of saddle sores/chafing. Underwear is not normally worn as there are seams in presaure areas and it is a layer that can shuffle about, causing friction, and bunch up. The norm is just the one layer of padding in your shorts and thats it whether you are 1 hour or 10hours in the saddle.
Sometimes creams can help in areas that are getting sore. They enable material to slide over the skin if that is what is happening. Also; hair that is being sat on might need addressing as it can be pullrd this way and that causing irritation. Short is good!

There are thousands of saddles and padded shorts. It is just a case of finding a combination that suits and some people are quite happy with unpadded old school saddles and minimal padding in shorts. Some people it is the other way around and the requirements of women and men can be very different, too.

Re: track & skinsuits. It isn't normal practice to wear extra padding underneath a skinsuit on either the track or during a time trial, apart from bra or heart rate monitor, as this could create extra drag resulting in reduced aerodynamic efficiency. It's also usual to wear skinsuits at least one size too small - if you can stand up straight in one *it it's too big! Wrinkles/bunched fabric cause drag. The materials used are pretty technical (designed to maximse airflow) and some of the more expensive ones, e.g., Endura can cost $599, Castelli $300-400.

Even though Rhys's events are short, every watt saved (not lost in increased drag) counts.

*I'm wondering if perhaps (having not seen the photo) if the rubber band might not be a heart rate monitor that has slipped down. If you actually have breasts, and wear a bra, it usually stays put under the bra!
