I was watching a dramaslop video, and happened to be actually looking at the screen while Ian was speaking, and I noticed something interesting: he appears to have genuine left-sided muscle weakness in his face. This is not uncommon in contact sports, and it is notable that it was not present prior to his short-lived boxing career. Unilateral facial weakness is usually caused by damage to Cranial Nerve VII, also called the Facial Nerve.
You can see that the left corner of his mouth largely remains in a fixed position, the left top lip no longer micro-adjusts to match expression (leading to Ian speaking/emoting much more from the right side of his mouth), and that the muscles on the right side are much more active, precise, and toned than the muscles on the left. You can see this also in the drooping of the left eye. This lends a lot of credence to the TBI/CTE musings.
