The “medical issue” could be Kayla is off her meds or is under medicated or all the booze from the hot tub party got her out of whack or from Nick’s perspective, she’s acting crazy. Maybe they’ve been fighting constantly & he’s had to keep an eye on her to prevent her from self harming or something like that. He only made the one comment about sometimes when you’re a dad you have to do thing like that (stay up all night with “someone”) but in the Elissa clips video of the Locals stream he said all the kids are “fantastic”, they are all doing fine. What if he meant sometimes when you’re a dad & you really piss off the mom you have to make sure she isn’t running off in the night while you’re sleeping or streaming & taking the kids with her? Sounds like he thinks he’s smoothed things over with her for the time being but it could pop off again at any time. Notable that in all his rambles he’s not mentioned her.
 
Addicts lie on their children all the time. They scapegoat their kids as cover for their addict bullshit behavior. Nick is smart enough to be vague about it, that's all. If you've ever personally known an addict or worked with one, you will be amazed by the lies they make up about their family incl kids as a way to deflect from their poor performance or other issues.

That last comment by Gym confirms what many were thinking. He truly hates Ralph and never forgave him after he went after Jade. Josh getting more clout with LOCALS and/or RUMBLE would really upset him. Like his fundraiser's failure in comparison to Josh's, it would be another mark of decline. Nick used to be the gatekeeper of access and pronouncer of diktat. He has lost that position rather spectacularly.
