I think it's clearly intended to be an adult woman. The biggest problem is that is looks like, and most likely is, bestiality fetish porn. For some reason, Patrick is so proud of this that he found it prudent to both defend and repost it multiple times despite being directly criticized for it. Then again, he's so unbelievably dumb that he may just be doing it as an act of defiance and is just too dense to realize it looks like something he peruses regularly, especially when he acts this way.

Technically, he is, but "fat" just isn't the word to capture his uniquely enormous girth. Calling him "fat" is like calling Mount Everest "big": technically correct, but it just fails to capture the monumental nature of it. It's difficult to find the words to capture his elephantine size, his ever-expanding excess adipose, his innumerable chins, his forever growing girth.
