Most likely what happened is the kid has damaged testes and was diagnosed as hypogonadal, and he was prescribed androgens to go through puberty normally, because you know, it's not healthy to not go through physical maturation.
There's this logical inconsistency with the spectrum argument that I'm finding it hard to articulate ,but I think it essentially boils down to how TRA's only want sex to be a spectrum when it comes to their self definitions, not when it comes to things like speaking in generalities about "women's" experience, or the predatory tendencies more prevalent in men. Because ultimately, there playing a sort of shell game where they substitute the sex as spectrum argument for an argument actually that sex is binary but only in specific cases. To gain access to sex segregated spaces, they are claiming that they are excluded from the group that makes those spaces necessary. But, that's only possible if you accept the idea that sex is a hard binary and that their self identification as women excludes them from all of the categories that could make them dangerous. However, if sex is indeed a spectrum, then logically they could still be women and share 99% of the traits (increased body mass, male socialization, a penis) that makes them a danger to other women, and so should be excluded on those grounds. So the trick being pulled is taking this sort of philosophical definitional argument about how the human mind labels concepts, and use it to argue against empirical observation that have really nothing to do with the labels we apply to them. You could say that trans women are women, but say that trans women and men belong to the category of smerp, defined by having a penis at some point in their existence, and exclude trans women from all antismerp spaces on the basis that most rape involves an individual who has or had a penis. Or you could just say that trans women are men and exclude them on that basis. But the effect is the same, and relates the same empirical fact that do not care what you call men and women.
