It is a kind of internal physical abuse, so I'm not sure how you'd exclude that or where you'd draw the line. Though yes, there have been cases of paedophiles raping infants and the sheer size difference and physical immaturity and weakness means they die from their injuries. Even for adult victims, internal organs get fucked up, they lose blood, it's certainly possible to die from rape.

Then the answer is probably no. Unless you are going to take into account the possibility of STDs like Hep and HIV. But even those diseases are dubious in that it doesn't guarantee a death. Intestines can be damaged so badly they may need to be surgically repaired or removed. Vaginal injuries are common bc a woman needs to be aroused to be penetrated without harm.
